Enrollment and Registration
Students residing in the Evergreen School District (ESD) are assigned an elementary and a middle school based on their home address. Find your neighborhood school. All enrollment takes place directly at your home school of residence. Please contact your home school of residence regarding their registration process.
Enrollment at the school of residence is on a first come, first served basis. Every attempt will be made to place new students at their school of residence. However, the district cannot guarantee such placement and reserves the right to overflow students to other schools within ESD as necessary when a grade-level enrollment capacity is reached. Any student overflowed to a school other than their school of residence will be provided transportation from their school of residence and back. Overflowed students will return to their school of residence the following school year or as space becomes available.

TK and Kindergarten Registration

Kindergarten Requirements1. The child must be 5 years old on or before September 1 (of the current year). 2. Parents and child reside in the Evergreen School District attendance area. NOTE: Registration packets must include proof of residence. 3. An original birth certificate with seal or other legal document showing date of birth must be presented to school personnel. 4. Record of UP-TO-DATE IMMUNIZATIONS must be presented to school personnel. If immunizations are not up to date, the registration will not be considered complete. Schools in California enforce a “no shots, no school” policy. The child WILL NOT be allowed to attend school if the immunization verification has not been received before the first day of school. 5. All Kindergarten students must be evaluated for risk factors for TB as part of their routine medical “assessment.” A Tuberculin Skin Test (TST ) or other TB test (IGRA blood test) approved by the Centers for Disease Control and Prevention will be ordered by the HCP if deemed necessary based on the TB risk factor assessment. The Santa Clara County Public Health Department TB Symptoms Review and Risk Factor Assessment Form which will be completed by the HCP will be returned to your student’s school, along with the registration packet. The student’s HCP will give parent/guardian documentation stating one of the following within 1 year of registration: a. Student’s risk assessment was negative. No TB testing is required. Students with TST/IGRA test results within 1 year of registration may submit their results with their registration packets and do NOT need to obtain the Assessment of Risk Factors from their HCP. 6. Students are required to have a dental check-up by May 31st of their kindergarten year. Oral evaluations completed within 12 months prior to school entry also meet this requirement. 7. Each child should receive a physical examination 18 months prior to entrance into the first grade or within 90 days after first grade entrance. Physicals must be given after March 1 (of the current year). |

Residency VerificationProof of residence is required for registration at ESD schools. Acceptable Proof of Residency Requirements are listed on the D-90G Residency Verification Declaration, which is included in the online registration packets (2019-20 Registration Information and Forms link above). A school which has been designated as an Impacted School may have additional proof of residency requirements. These requirements can be obtained from the school office of the Impacted School. As of 2008-2009, Carolyn Clark and Matsumoto Elementary Schools have been designated as Impacted Schools. |
School Choice Transfer RequestThe parent/guardian may request that a child attends a school other than their home school within ESD through a School Choice Transfer Request. Interdistrict Transfer RequestChildren residing outside ESD may request enrollment at a school within ESD by first starting the Interdistrict Transfer process at their home school district. Approval must be obtained from the home school district as well as from ESD prior to enrolling at an ESD school. ESD approvals of Interdistrict Transfer Requests are based on space availability for Interdistrict Transfer Requests and require an annual renewal. Parent/guardian is responsible for the transportation of students approved through the Interdistrict Transfer Requests process. |
