Heal360 Update on Insurance Process

Note: This change only applies to ESD students/families & community members who choose to be tested with HEAL360 Testing Center located at Dove Hill School. The insurance information is being requested by HEAL360, NOT Evergreen School District. This change does NOT apply to Concentric Pool Testing in elementary schools.

Heal 360 and Evergreen School District have formed a partnership to support the safeguarding of staff, students, and the community against COVID-19. Due to the COVID-19 pandemic, the Cares Act was enacted after the federal and state governments declared a public health emergency. The goal of this federal legislation is to provide services for the health and safety of the public. The legislation was put in place to ensure all individuals could receive testing and vaccination without costs. As of Wednesday, March 23, 2022, the federal funding for COVID-19 testing will no longer be available ( Additionally, funding for vaccinations will not be available after April 5, 2022. Heal 360 will do its best to make this transition as simple and easy as possible. Individuals who are uninsured will be required to pay for COVID-19 testing at the time of service. Individuals with health insurance coverage will not be affected, however, HEAL360 requires proof of health insurance coverages, through private health carriers or MediCAL.

What you need to know if you or any member of your family choose to get tested with Heal 360 at located at Dove Hill School...

  • Students who do not have insurance registered with Heal 360 OR have not brought a copy of their insurance card will not be tested.
  • To submit insurance information: Email insurance information; insurance provider’s group number, policy number,  and your child’s name and birthdate to, cc:, cc: 

Individuals (i.e. parents, siblings of students & other community members) and students whose insurance information is not on file with Heal 360 can also register their insurance information using a new registration link. The registration link will be provided the week of March 28, 2022, please stay tuned.  The link will be sent via ParentSquare.

For those who do NOT have health insurance coverage:
A $55.00 per test fee applies.

  • Either a rapid antigen test or PCR test can be provided
  • Both tests can be conducted for a total cost of $100 ($50/each)

Individuals will scan a QR code on-site to confirm the appointment and submit payment information.
