Residency Verification

Three proof of residence documents are required for registration at Evergreen School District schools.

*A school which has been designated as an Impacted School require additional proof of residency requirements. Carolyn Clark and Matsumoto elementary schools are designated as Impacted Schools. These two schools require 4 proof of residency.

Acceptable Proof of Residence Forms

  • California Driver's License
  • California Identification Card
  • DMV Boat or Car Registration
  • Escrow Papers
  • Property Tax Bill
  • Home Owner/Renter Insurance Policy
  • Lease/Rental Agreement
  • Utility Bills - PG&E, Water, Garbage
  • Home Phone Bill or Cable Bill (Cell Phone Bills are not acceptable)
  • Government Agency Identification/ Correspondence:
    • Income Tax Return
    • W-2
    • Registrar of Voters - Proof of Residency
    • Jury Summons
    • Unemployment
    • Other
  • Evergreen School District / Federal Funded Program Confirmation (i.e. Migrant Program)
  • Monthly Payments:
    • Loan
    • Mortgage
    • Credit Card
    • Insurance
    • Rental Payment - Cancelled Check
  • Payroll Check
  • Bank Statement
  • Physician / Dental Bill

Original documents must contain name and address for proof of residence.

**Current proof of residence documents are the most recently issued documents (i.e. monthly statements are received within the last 45 days.)  Institution-issued statements must be provided.

The District has the right to accept alternative forms of proof of residence on a case by case basis.
