Parent and Student Confirmation Form
California Education Code section 48980 requires that, at the beginning of the first semester or quarter of the regular school term, the governing board of each school district must notify parents/guardians of their rights or responsibilities under certain provisions of the Education Code. Other provisions of California and United States law also require notification of parents/guardians. This information is available in the menu located in the right column of this webpage.
Education Code section 48982 requires acknowledgement of being informed which is performed by physically or electronically signing the parent acknowledgement card to indicate that you have received the notice and have been informed of your rights.
We request that all parents in Evergreen Elementary School District sign an online Parent and Student Confirmation Form.
How do I get started?
If you have an existing PowerSchool Parent Portal account, you can log in here. Once logged in, click on 2023-2024 Parent and Student Confirmation on the left side bar.
The Annual Parent Acknowledgement and Consent Form will need to be reviewed by the parent/guardian and returned with each student within one week of receiving the handbook.
